Tracy Christner is the Executive Director of Empath Choices for Care, a member of Empath Health. She provides professional and community education about healthcare decision making, as well as individual and family advance care planning services. Her special focus is addressing disparities and removing barriers in the use of advance directives in the African American, Hispanic and LGBT communities.
Christner is a certified facilitator/instructor in the First Steps (Respecting Choices®) and Last Steps® (POLST) Advance Care Planning curriculums of Gundersen Lutheran Health System. She serves on the executive committee for the Florida POLST (Physician Orders for Life Sustaining Treatment) initiative and is a frequent speaker on the subject at national conferences. She also is a member of the Health Equity Committee of the Tampa Bay Healthcare Collaborative.
Community Outreach Counselor
Arwyn Elden, LCSW is the Community Outreach Counselor for Empath Choices for Care, a member of Empath Health. Elden provides professional education, community outreach and information, and individual or group assistance with creating living wills.
Elden is a certified facilitator/instructor in the First Steps (Respecting Choices©) and Last Steps (POLST) Advance Care Planning curriculums of Gundersen Lutheran Health System. She has been involved in advance care planning since 2000. Her experience includes clinical work in hospice and hospital settings, as well as public education and teaching. Elden holds a Master’s degree from Bryn Mawr Graduate School of Social Work and Social Research.
